Continuum Care Centres
Sukino’s continuum care centres are envisioned as an intermediate care facility between the hospital and the home and provide the right medical and psychosocial environment for such patients to recover and rehabilitate.Each continuum care centre is a fully-furnished, 15-30 bedded residential facility with 24-hour skilled nursing and physiotherapy unit that offers both short-term as well as long term care. They are conveniently located in close proximity to many major hospitals. This allows for our patients to easily move in from a hospital environment to a home-like environment for recovery. The proximity to the hospitals also allows us to engage with the hospital in case of any emergencies or follow-up review with specialist consultants.
Upon admission, our multidisciplinary team of professionals develops a care plan tailored to the needs of the patient. Team members coordinate the various therapies, including physiotherapy, mobilization and diet for each individual patient, and constantly monitor the patient for any on-going medical issues.
All our private rooms are designed to accommodate one family member with every patient, with comfortable attender beds and a range of amenities for their comfort, including large-screen HDTVs, complimentary Wi-Fi and laundry. Additionally, they have access to common lounge areas and dining areas. We strive to make our continuum care centres your home away from home.
